I really tried to like these shoes because they do feel great out of the box and I found the range on the boa closures to clamp down pretty snug on my low volume forefoot. I also wear Sidi Narrows Genius 5 to give you an idea of how narrow my feet are and I wear both Specialized and Sidis in a size 46. The S-Phyre fits well out of the box. Except! I had issues with comfort related to the threaded inserts and sole profile. These inserts essentially float loose and are held in by an adhesive fabric backer on the inside and these red rubber spacers visible in the pictures of the soles. This fabric backer detached from the insole after two rides and the inserts essentially fall out when not attached to a cleat. Also, the red "spacers" pictured on the sole are essentially useless. Those don't stay in although I am not sure they serve a purpose anyway. It makes cleat replacement very tricky because you have to hold the insert in place from the inside while you attach the cleat. In addition you need to continue holding the insert in place until you have tightened the cleat screws enough to be sure they stay in the channel that is carved from them. This brings up the second issue, with nothing significant between the insert channels and the insole (fabric backer or not), I found the channels could be felt through the insole and essentially caused serious hot foot for me. These just aren't for me and they have been relegated to expensive "spin class" shoes.
