I recommend the Large

At first, I was thinking, eh, this isn't the best, it's lumpy and uncomfortable, and it makes weird rustling sounds in my ears from the form compressing/decompressing--then I used it a couple more times. Now, I'm really into it. Seriously, I have no problem trading out my bed pillow for this pillow. I think the first night I was just thinking too much about the pillow--it was a "princess and the pea" situation.
I recommend the large. The pillows really don't take up all that much room regardless of size--I squish mine down smaller in a compression sack. The foam really compresses very well. I own the medium and the large and I like the large better (I'm a side/stomach sleeper). The large has more foam and provides better support. I don't think the medium compresses all that much smaller and comfort benefit of the large is very worth it. Note: if you're using it INSIDE the hood of a mummy bag you may have different preferences--I camp in pretty warm climates...
