I'd go large. Patagonia tends to fit a bit small, and you're right at the upper end of medium anyway. Also, be sure to measure properly for your arm. You should measure from the center of the back of your neck, along the shoulder, down to your elbow (which should be bent about 90 degrees) to the top of your wrist.
