I currently own a pair of Makalu's that I've worn for about 10 years now, however my feet have flattened out over the years and my boots are quite snug now. I wear a 11.5 street/running shoe, so should I go for the euro 45 or 45.5? Sizing says to go 1/2 euro size up if you are between sizes with running shoes.

I'm the same as you in terms of foot size - 11.5 running shoe with wider, flat foot. I got the 45.5 Makalus. They fit perfectly with your standard hiking/mountaineering sock.
