What size bladder would you recommend for this pack? Also...would you recommend insulation for the tube and/or bladder?

I'd use the CamelBack 70oz antidote reservoir with a neoprene sleeve for insulation. Also, after taking a sip I blow back into the feed tube to push all liquid back into the bag. Never had any freezes this way,even at -20 degrees.

I use a 3L platypus but it's too long and doesn't fill the width but it works. I've had mine in the teens with out freezing the bladder or tube by stuffing it back into the shoulder cover.

I'd go for a 3L bladder. You really could go either way with the insulation for the tube, but I don't think its necessary for the bladder unless you're going to be in a place thats really really cold - the tube, insulated or not, will almost always freeze up before the bladder. That being said, I've gotten away with just a tube in the shoulder sleeve down to the low teens.
