My head measurement is 22. So..........do I buy the Small or the Medium. Please don't suggest that I wear a balclava underneath the Medium to snug it up. I have a thick head of hair and always have the vents open for ventilation.

22 inches is 55.8 cm. the small is 54-56cm. That would mean that you are a small. This helmet is adjustable so you might be able to wear the medium. However, you need to try on helmets to make sure that they fit. Not all heads are shaped the same. A helmet in your size may not fit properly because of the shape, even if a helmet of the same brand and size fits well.
