Is there a difference between the medium size and the large? How is the head measured? If I buy the large, would it be too big and fall off?

I wear a medium and my head measures about 7 1/3. Most people will wear a medium but if your head is larger than normal I'd got with a large. If it's a bit bit it's not going to fall down. Just won't be quite as snug.
