Smith does not make a prescription lenses for Thresholds. I even took my pair in to a local shop that does custom lenses and even they could not do it due to the design of the Threshold frame. I know it sounds like a great idea to swap in a prescription lenses but I just don't think it's practical. I like Smith sunglasses but for prescription I'd go with Oakley. Smith customer service was not very helpful to me at all despite the fact one is able to speak to a live person fairly easily. I got the impression that they could really give a rat's ass about helping me find a prescription pair of their glasses.

To my knowledge yes you can, but you would have to have Smith make you a prescription specific lens. I do know that they do such a thing. I would just give them a call at 800-635-4401 and see what they can do for you.
