Yes, the cap extends down to cover the ears, atleast most of them. Mine comes down to about the bottom of the ear loob, sorta where your helmet strap connects under the ear. It is toasty warm too.

I must say that this cap works, it does block the wind and keeps your ears warm. But it feels like a shower cap on my head it also makes krinkle noises like a shower cap I wish backcountry had the gore Hot Noggen it works the same but is more comfortable and quieter.

Medium should work fine although if your helmet has a lot of fins in the back, you might want to go with the largeDave - GORE BIKE WEAR(tm)

If your head ever gets cold in the mornings or night. If your ears tend to freeze like ice. This will cure that the second you put it on. SO WARM! Easily the warmest cap I've ever worn. Make sure it is cold out though or this will probably be TOO much! It small though so I guess you could just pull it off and put it away!
