Yes, it does.

Worked Great

I didn't notice the water flow being slower. I was impressed.

The perceived flow will depend more upon the vertical distance from the mouth piece to the reservoir than it will on the hose length.

If the reservoir is on a significantly lower plane than the bite valve it will take a significant amount of "sucking" power to get the fluid up the hose. Where as if the two are on the same plane drinking should be nearly effortless regardless of the length of the hose.

would this tube work as a total replacement tube with mouthpiece?

Love it. Makes it a bit harder to get water out. I run it from the trunk bag on my bike, along the frame and to the handlebars to make sipping easily available.

does what its supposed to

it makes a good extender or replacement tube. my tubes get moldy long before the hydration bladder, so this is the best way to replace the tube. just put the end in boiling water for a few minutes, and it will easily fit over the valve on the hydration bladder.

Do you need it?

Not necessary unless you need a very long extension. Also should come with insulation to keep hose from freezing.
